The Department of Technology Infrastructure provides these forms to establish a set of standard operating procedures for all schools participating in the 1:1 program. These documents are not intended to be edited and should be used as-is by all schools.
Schools must ensure that all families are informed of the Technology Repair & Replacement Costs. A parent or guardian needs to sign the Electronic Device Usage Agreement for each Chromebook transaction. The form should be signed electronically in TipWeb-IT by generating the Full Distribution Receipt when the Chromebook is issued. Any paper copies of the form must be scanned and attached to the student record in TipWeb-IT.
